Request FormsFor your convenience, here's a list of on-line request forms faculty might need in preparing online instructional materials:Blackboard Request Forms Use these forms to make adjustments to your Blackboard site, such as copy materials from a previous course or add a TA to the course. Go to ITD's Blackboard page to find more information on Blackboard. Audio Visual Services Request Form Use this form to request a DePaul sponsored event or academic course recording. Turn It In Account Request The plagiarism detection service, Turn It In, is provided for use by DePaul faculty. Streaming Media Request Form Use this form to request an account on DePaul's Streaming Media server. The Streaming Media Server provides DePaul faculty with the ability to incorporate audio and video in their Blackboard courses. Mailing List Request Form Use this form to request a Mailman e-mail discussion list for academic purposes. Classroom Reservation Request Form Use this form to reserve a computer lab for class sessions. Condor Account Request Forms Use this form to request an individual account on DePaul's faculty web server. To request a departmental web site use the request form at: http://is.depaul.edu/resources/forms/departmental_website.asp. General Request Form Use this form to request assistance with any issue related to technology and teaching.
